RESUMEN

In April 2021, the Japanese government decided to discharge nuclear wastewater from the Fukushima Daiichi nuclear power plant into the sea, which aroused widespread concern all over the world. The radioactive substances in nuclear wastewater may affect the environment through the action of ocean circulation, groundwater or carbon cycle. In addition, it may expose people to the health effects of low-dose but long-term pollution once nuclear wastewater is discharged into the sea. Based on this, this paper discusses the possible global environmental impact of nuclear wastewater from a general perspective and illustrates its possible health effects as well as potential mental health problems from an individual perspective, and puts forward some suggestions on the issue of Fukushima nuclear wastewater discharge.

RESUMEN

ObjectiveTo explore the mental health status and relevant factors among community residents during the normalization stage of prevention and control of COVID-19. MethodsFrom August 28 to September 7, 2020, an online cross-sectional survey using snowball sampling was conducted among community residents via Wenjuanxing platform, and their mental health status were assessed using Patients' Health Questionnaire Depression Scale-9 item(PHQ-9), Generalized Anxiety Disorder-7 item(GAD-7), Insomnia Severity Index(ISI) and Chinese Perceived Stress Scale(CPSS). ResultsAmong the 476 community residents, the detection rates of depression, anxiety, insomnia and high perceived stress were 32.35%, 21.22%, 24.58% and 48.74%, respectively. In terms of gender, the detection rate of high perceived stress was higher in male than in female(χ2=5.269); in terms of marital status, the detection rates of depression and anxiety among the unmarried, divorced or widowed residents was higher than those of the married residents(χ2=5.251, 8.851); in terms of mental health service status, the detection rates of depression, anxiety, insomnia and high perceived stress among residents with mental health service needs was higher than those among residents without the needs(χ2=46.316, 66.934, 20.153, 21.576), with statistical significance(P<0.05 or 0.01). Correlation analysis showed that the age of community residents was negatively correlated with CPSS score(r=-0.171, P<0.01), sleep duration was negatively correlated with PHQ-9, GAD-7 and ISI scores(r=-0.210, -0.247, -0.297, P<0.01), and time spent following news on COVID-19 per day was negatively correlated with ISI score(r=-0.097, P<0.05). ConclusionDuring the normalization stage of prevention and control of COVID-19, the majority of community residents experience the symptoms of depression, anxiety, insomnia and high perceived stress. Male and younger residents are more likely to experience high levels of perceived stress. Those with shorter sleep duration and needs for mental health services are more likely to experience symptoms of depression, anxiety and insomnia, and those with short time spent following news on COVID-19 are more likely to experience insomnia symptom.

Objective To OxplorO thO opOrating mOthods and clinical OffOcts of arthroscopic anatomical antO-rior cruciatO ligamOnt(ACL) rOconstruction with dirOct insOrtion tOchniquO vOrsus traditional mOthod.Methods From January 2017 to DOcOmbOr 2017,totally 52 patiOnts who accOptOd ACL rOconstruction and mOt thO inclusion and Oxclu-sion critOria in BOijing FriOndship Hospital of Capital MOdical UnivOrsity wOrO OnrollOd in this rOtrospOctivO study. ThOy wOrO dividOd into two groups by random sOquOncO softwarO and patiOnts′sOlOction rOsults. ThO obsOrvation group (27 casOs) rOcOivOd dirOct insOrtion tOchniquO,and thO control group(25 casOs) rOcOivOd traditional mOthod. At post-opOrativO 1 month, 3 months, 6 months, 1 yOar, thO clinical OffOct was OvaluatOd by imaging indOx, subjOctivO and objOctivO indicators.Results All 52 patiOnts wOrO availablO for follow up with an avOragO of 11.5 months. No postop-OrativO complications such as infOction, joint instability and soft lOgs, with a satisfiOd rOcovOry of rangO of motion, pain rOliOvOd significantly. ThO Lysholm scorO of thO obsOrvation group was (94.80 ± 4.18)points, which was signifi-cantly highOr than (91.02 ± 1.96)points of thO control group (t=2.674,P<0.05).Lachman tOst and KT-1000 wOrO improvOd significantly comparOd with thosO bOforO opOration ( all P <0.01), whilO thOrO was no statistically significant diffOrOncO bOtwOOn thO two groups(all P>0.05),but thO pivot-tOst of thO obsOrvation group was bOttOr than that of thO control group, thO diffOrOncO was statistically significant ( P < 0. 05 ). ThO ACL anglO of thO obsOrvation group was (51.52 ± 5.18)°, which was significantly lowOr than (55.86 ± 2.45)° of thO control group ( P<0.05). According to modifiOd Lysholm scorOs classification, in thO obsOrvation group, 24 casOs wOrO OxcOllOnt, 2 casOs wOrO good,1 casO was fair, and thO OxcOllOnt and good ratO was 96.3%. In control group,18 casOs wOrO OxcOllOnt,5 casOs wOrO good,2 casOs wOrO fair, and thO OxcOllOnt and good ratO was 92.0%. ThO diffOrOncO in thO OxcOllOnt and good ratO bOtwOOn thO two groups was statistically significant(P<0.05).Conclusion DirOct insOrtion anatomical rOconstruction is a safO and OffOctivO tOchniquO for rOconstruction of ACL. It can rOstorO thO stability of ACL and rotation of knOO joint.

RESUMEN

Objective To investigate the expression of Shh, Ptch, Smo and Gli1 proteins who belongs to components of Hedgehog signaling pathway, and to explore the relationship between the abnormal activated of Hedgehog signaling path?way with renal clear cell carcinoma (RCCC). Methods Protein expressions were examined by immunohistochemistry in 76 cases of human RCCC and 12 pericarcinomatous tissues. And 10 fresh specimens from cancer tissues and pericarcinoma?tous tissues were used to examine expressions of Shh, Ptch, Smo and Gli1 by Western blotting. Results The expressions of examined proteins were higher in RCCC than that in pericarcinomatous tissue. Shh (52.6%vs 8.3%), Ptch (63.2%vs 16.7%), Smo (44.7%vs 8.3%) and Gli1 (60.5%vs 25.0%). The increased proteins levels were disassociated with age, gender, and tu?morous size in RCCC. But it was significantly correlated with TNM stage of RCCC. The expressions were gradually increased with higher TNM stage of RCCC or more invasive malignancy. Conclusion Hedgehog signaling pathway is highly activated in RCCC. It might be involved in initiation, progression of RCCC, and it might be used as the target for RCCC treatment.

RESUMEN

Objective To investigate the clinical application of the combined the anterior malleolus flap and anterior tibia flap. Methods Based on the dissection of the perforating branches of anterior tibial artery on the middle and inferior section, the combined the anterior malleolus flap and anterior tibia flap was designed to repair the necrotic skin of anterior foot for 5 patients. The sizes of the flaps ranged from 17 cm×10 cm-10 cm× 5 cm. And the area of the flap was from tibial tuberosity(upper bound) to the line between internal malleolus and external malleolus (lower bound), and from the median line of one side of leg to the other side. Results Postoperatively, all flaps survived, and the primary healing of transplanted skin in donor site was achieved. The texture of flaps were excellent, the phenomenon of abrasion did not happen, and the clinical therapeutic efficacy was satisfactory after a follow up of 2-24 months. Conclusion It's a good method that combined the anterior malleohls flap and anterior tibia flap, which not only could enlarge the area of the flap but also has reliable blood supply, in repair of large size skin defect of anterior foot.

Objective To investigate effects of angiotensin converting enzyme (ACE) gene insertion/deletion (I/D) polymorphism on endothelial function in elderly patients with essential hypertension. Methods Seventy-three elderly patients with uncomplicated essential hypertension and 55 normal elderly people as control were recruited into the study. Their humeral arterial endothelial vasodilatation function was studied with high-resolution ultrasonography and ACE I/D genotypes were determined by polymerase chain reaction (PCR) technique. Results Endothelium-dependent vasodilatation function of the three-genotype subgroups of the elderly patients with essential hypertension was significantly lower than that in the controls with the same genotypes (5.5±1.9 vs 11.9±1.3 in Ⅱ genotypa, 4.7±2.0 vs 10.9±1.6 in ID genotype and 2.9±1.9 vs 9.4±2.6 in DD genotype, with all P<0.01 ). In both groups of hypertensives and normotensives, humeral arterial endothelium-dependent vasedilatation function was significantly lower in those with DD genotype than that in those with Ⅱ genotype (2.9±1.9 vs 5.5±1.9 in hypertensive group and 9.4±2.6 vs 11.9±1.3 in control group, both P<0.05 ). Multivariate linear regression analysis showed that endothelium-dependent vasodilatation function in hypertensive elderly people correlated in linearity with their ACE genotype (P<0.01 ). Conclusions ACE gene I/D polymorphism is associated with abnormal humeral arterial endothelium-dependent vasodilatation function of humeral artery in patients with essential hypertension.

RESUMEN

<p><b>OBJECTIVE</b>To assess the relationship between myocardial regional perfusion using second harmonic myocardial contrast echocardiography (MCE) by venous injection of Levovist and coronary artery stenosis detected by coronary angiography to determine whe ther MCE can be used to detect coronary artery disease (CAD) and its sensitivity and specificity for detecting CAD.</p><p><b>METHODS</b>Thirty-six patients who underwent coronary artery angiography and MCE formed the study groups. Ten myocardial segments (5 each in the apical two- and four-chamber views) from the images were scored for detecting myocardial perfusion as follows: 1, normal perfusion; 2, decreased perfusion; and 3, perfusion defect. The arteries were classified as normal or diseased. The diseased arteries were classified into three groups according to the perfusion scores.</p><p><b>RESULTS</b>There were significant differences in coronary diameter stenosis among the different perfusion score groups (P < 0.001). There were 10 total occluded arteries, and the myocardial perfusion scores were different because of different collateral circulation. In the normal perfusion group (Group A), the coronary diameter stenosis was 65% +/- 12%, and the myocardial perfusion score index was 1 +/- 0.00. In the decreased perfusion group (Group B), the average coronary diameter stenosis was 82% +/- 8%, and the myocardial perfusion score was 1.93 +/- 0.16. The diameter stenosis was less than 85% in 63% of the coronary arteries (including diameter stenosis < or = 75% in 12% of the vessels). The diameter stenosis was 85%-90% in 22% of the coronary arteries and > 90% in 15% of the arteries. In the perfusion defect group (Group C), the average diameter stenosis was 90% +/- 6%, and the myocardial perfusion score index was 2.89 +/- 0.24. The diameter stenosis was > or = 85% in 94% of the coronary arteries, and the diameter stenosis was < 85% and > 75% only in 6% of the coronary arteries. The overall sensitivity and specificity of MCE in identifying angiographic coronary diameter stenosis was 67% and 100%, respectively. The false negative rate was 32.6% for the 108 coronary arteries. Further subdivided analysis showed the sensitivities in Groups A, B and C were 0, 100%, and 100%, respectively. The sensitivity increased with increased lumen diameter stenosis of coronary arteries.</p><p><b>CONCLUSIONS</b>There is a close relationship between coronary artery stenosis and MCE perfusion scores. MCE with venous injection of new generation contrast can define the presence of CAD and lesion graded classifications. Some disagreements between perfusion score and coronary diameter of stenosis may indicate other factors such as different collateral circulation, which should be further investigated. As artery stenosis increases, the sensitivity of MCE is increased.</p>
